The Mary Jo Dudley Diabetes Care Program was originated by Mary Jo Dudley, RN, CDE in 2002. She is an esteemed diabetes educator, diabetes advocate and founding member of the Granite State Diabetes Educators (GSDE) professional organization. Originally titled “Granite State Diabetes Care Connection,” the Program was renamed in honor of Mary Jo in 2015 for her dedication and contribution in the field of diabetes. The Mary Jo Dudley Diabetes Care Program was developed to assist the medically indigent with diabetes supplies and services, when traditional funding sources are exhausted or absent. It is only used as a “last resort” resource in the absence of personal resources, public assistance, prescription assistance and/or commercial health benefit coverage.
Criteria for Eligibility
Assistance is available to New Hampshire residents regardless of race, creed, religion, sexual orientation, or country of origin who meet the following criteria:
It is assumed that the active GSDE member, working with the client, has already determined the most cost effective medication, insulin or testing regime within the client’s financial constraints. The Program may only be accessed once per client.
Covered Products and Services
The Program will cover products and services essential to the well-being of the person with diabetes in order to effectively manage the diabetes condition:
Any collateral expenses, such as: service fees, transportation, dependent care, co-payments
